We are a collection of talented professionals all committed to providing the best possible services to our patients. Standing firm in the footsteps of Mid South Rehab, we promote integrity, efficiency, and effectiveness in all of our work on a day-to-day basis. With this model, we are able to restore patients to their highest possible functional levels.


Our staff is made of licensed professionals, and the majority of our Physical Therapists possess doctorates in their field.  Each therapist also meets both state and national requirements for his or her position.  


Director of Rehab

John Robert Burnett, “JR”, is a Doctor of Physical Therapy and is the Director of Rehab at 360 Total Rehab in Flowood, MS.  JR graduated from the University of Mississippi Medical Center in 2014.  While at UMMC he specialized in clinical rotations in neuro-rehabilitation in Memphis, TN, outpatient spinal rehabilitation at New South Neurospine, Acute Rehabilitation in Oxford, MS, and general outpatient orthopedics in Jackson, MS.  


JR’s received his Bachelor’s in Business Administration from Millsaps College in the Spring of 2011 where he graduated with high honors.  He also played varsity football at Millsaps lettering all 4 years and making the leadership council of the team.  He began his career as a physical therapist with Mid South Rehab upon graduation from UMMC and worked primarily in the outpatient department at Magee General Hospital.  In June of 2015 JR made the transition to Director of Rehab at 360 Total Rehab in Flowood, MS.  He has since earned an advanced certification in the performance of Pre-employment screenings and Functional Capacity Evaluations through Workwell, a nationally recognized leader in workplace training and education for physical therapists.  


JR has also been an integral part of the development of a partnership formed between 360 Total Rehab and Hartfield Academy where JR serves as a teacher and a coach on the football team utilizing his knowledge and talents as a physical therapist and his love of Jesus Christ to assist in the development of the physical, mental, and spiritual development of the students of Hartfield.  JR has been married since 2012 to his beautiful wife Kelsey and they have a 3-year-old daughter and a 1-year-old son.  

Above all else JR wants everyone to know that, “I am a follower of the Lord Jesus Christ, and while family, work, education, and life experiences are integral parts of who I am, apart from Him it is all vanity.”


Physical Therapist

Kim Middleton, Physical Therapist at 360 Total Rehab, is a graduate of the University of Mississippi Medical Center and has worked as a PT in the outpatient setting for 20 years.  She has worked for Mid South Rehab at 360 Total Rehab since 2012.  


Kim has advanced certifications in the performance of Pre–Employment screenings and Functional Capacity Evaluations through Workwell, a nationally recognized leader in workplace training and education for physical therapists.  She has also completed course work towards becoming Board Certified to perform Dry Needling and should complete all necessary course work for certification in the first quarter of 2017.  


Kim enjoys running, playing the guitar and singing with her band, and leading the praise team at Metro Grace Community Church in Pearl, MS. She also enjoys riding her motorcycle and spending time with all of her pets, which she refers to as her “fur babies”!


Office Manager

Paula Hairston is the Office Manager at 360 Total Rehab in Flowood.  She is a graduate of Tougaloo College and she began working for Mid South Rehab in 2010.  


Paula started her career with Mid South at the Employee Support Center as a receptionist and later served in the medical records department at the Employee Support Center.  After a brief period away from the company, Paula returned to serve as a rehab tech at one our skilled nursing facilities and ultimately took her current position, office manager at 360 Total Rehab, in August of 2014.    


Paula is originally from Columbia, MO, but she and her husband have lived in Jackson for several years. She is the mother of four children and in her spare time she enjoys reading and playing the keyboard at Living Word Christian Church.


PRN Physical Therapist

Brandi is an experienced Physical Therapist in the outpatient setting with greater than 12 years of clinical experience.


She works at our 360 clinic on an as-needed basis and has a wealth of knowledge in a variety of outpatient treatment approaches including an advanced certification in the performance of Functional Capacity Evaluations and Pre-Work Screens.


We are committed to providing our Employee Partners the clinical resources and tools they need in order to be distinguished as the premier therapy provider of choice in the rehabilitation industry. We require all of our Employee Partners to attend annual training on the following topics:

  • Annual Medicare, Medicaid, Third Party Reimbursement, and Compliance Updates

  • OIG Updates

  • HIPAA Updates

  • CCI Updates

All Employee Partners also receive seven hours of CEU's for attending and Nursing Home Administrators and Nurses receive seven hours of CEU's as well. We also host quarterly training for our Directors of Rehab. 

Since we are committed to quality patient care, we have developed the following specialized clinical programs and protocols:

  • Aural Rehabilitation

  • Contracture Rehabilitation

  • Dysphagia Therapy

  • Incontinence Program

Our therapists are trained in the latest clinical pathways and protocols in order to achieve the most effective approaches to treating specific diagnoses.
